Acthar in the treatment of Post-renal transplant recurrent FSGS
Details
Drug or Biologic Kidneys Age: 18 - 99 years
Gender: Any
This research study is looking at the use of a drug called H.P. Acthar® Gel (Repository Corticotropin Injection) ("Acthar"). Acthar is currently approved by the Food and Drug Administration for the treatment of infantile spasms in infants and children under 2 years of age and for the treatment of exacerbation (worsening or increase in severity of disease) of multiple sclerosis in adults. Acthar may be used for the following disorders and diseases: rheumatic; collagen; dermatologic; allergic states; ophthalmic; respiratory; and conditions marked by swelling. Note: Acthar has not been studied specifically in transplant patients. The use of Acthar is thought to be safe as side effects are related to steroids (corticosteroids) which are commonly used in renal transplant patients as part of routine immunosuppression. The purpose of this research study is to find out if the addition of Acthar to the current therapy your doctor is using to manage your FSGS has beneficial effects on transplanted kidney function by decreasing over time the amount of protein being excreted through your kidney.

ACL Reconstruction and Biofeedback Study
Details
Drug or Biologic Bones, Joints, Muscles, Pain, Pediatrics, Teens Age: 16 - 35 years
Gender: Any
Individuals with a history of anterior cruciate ligament reconstruction (ACLR) are at a heightened risk for developing osteoarthritis. The heightened risk for developing osteoarthritis is likely caused by either too much force or too little force applied to the knee during tasks such as walking. In order to develop effective treatments that can change the force applied to the knee during walking, we must first determine what amount of force is beneficial for the knee following ACLR. The purpose of this research study is to determine if feedback about the force applied to each leg during walking can be used to alter force while walking. The feedback used in this study will display targets for average force, high force and low force. We will then determine the effect of average force, high force and low force on lower extremity movement patterns and markers of cartilage turnover and inflammation. You will be asked to complete 1 screening session (1.5 hours) and four testing sessions, with one week in between each session. Each testing session will last approximately 5 hours, (most of this time you will be sitting) therefore the entire duration of the study will be 21.5 hours over 5 weeks. If you choose to take part in the study you will complete the same assessments in the same order during each testing session. The assessments include: questionnaires, activity monitoring, blood draws, gait biomechanics assessment, and a walking biofeedback intervention.

Drug or Biologic HIV/AIDS Age: 18 - 64 years
Gender: Any
We are learning about the safety of combining a cell therapy (HXTC therapy) with regular doses of a drug called Vorinostat (VOR) as a possible approach for improving the strength of your immune system against HIV. In this study, we will collect your blood, remove the cells in your immune system that fight infection (T cells), and expose them to HIV proteins so that they will recognize the HIV virus again. These re-educated T cells are called HXTCs (HIV-specific T cells). In this study we will use HXTC therapy (putting your re-educated T cells back into your body to get your immune system to respond better against HIV) along with the drug VOR, which has been shown to stimulate your resting CD4 cells to become active and release the HIV virus. The purpose of this study is to: 1) Evaluate the safety of a series of HXTC infusions in combination with serial doses of VOR and 2) Help scientists evaluate ways of re-activating latent (non-active) HIV virus and determine if the immune system can be made stronger to eliminate the activated HIV virus.

Because you and others have experienced symptoms like these, local doctors are conducting the 1289-0032 Research Study. In this study, doctors want to evaluate the safety and effectiveness of an investigational drug and compare it to placebo, which looks like the investigational drug but contains no active medication. The investigational drug is not approved to treat any mental health or medical condition. The results of this study will provide more information about the investigational drug and whether it could one day be used to help people who have experienced symptoms similar to yours.

Lupus Brain
Details
Procedures Auto Immune Diseases, Brain/Head, Lupus, Mental Health, Pain Age: 18 - 65 years

Systemic lupus erythematosus (SLE) is a severe, chronic autoimmune disorder that affects multiple organ systems and it is common for patients with SLE to also have other symptoms, including depression, chronic pain, and cognitive impairment (i.e., "brain fog"). Previous research shows that reducing these may lead to better quality of life for patients but current treatments for SLE are inadequate and the development of new therapies has been slow. Targeted stimulation modalities, such as non-invasive transcranial alternating current stimulation (tACS), have been shown to directly affect brain activity and improve depression, chronic pain, and cognitive impairment in patients that do not have SLE. The purpose of this clinical trial is to investigate the effects of tACS on patients diagnosed with SLE to see if they would also see a similar improvement in depression, chronic pain, and cognitive impairment.

FLOW - effect of semaglutide v placebo on renal impairment in individuals with type 2 diabetes and chronic kidney disease
Details
Drug or Biologic Diabetes, Kidneys Age: 18 - 99 years
Gender: Any
Chronic kidney disease (CKD) and diabetes often co-exist and, for the majority of cases, the kidney damage and/or reduced kidney function is caused directly by longstanding and poorly controlled diabetes. Improved glycemic control has been suggested to reduce the progression of CKD in type 2 diabetes (T2D) and both glycemic and blood pressure control are key recommendations in international treatment guidelines for CKD in T2D. Yet there remains a major unmet medical need to improve the treatment of CKD in patients with T2D. The purpose of this trial is to demonstrate that semaglutide subcutaneously (s.c.) delays the progression of renal impairment and lowers the risk of renal and cardiovascular mortality in subjects with T2D and CKD.

This is a research study about vestibulodynia, a condition where women feel pain at the opening of the vagina. They feel this pain regularly when there is pressure in the vagina - pressure from using tampons and/or pressure from sexual activity. Vestibulodynia is very common - it affects one out of every six women. Through this study, we hope to find out which medications work best to relieve pain from vestibulodynia, and also find out more about what is happening in the body when women have vestibulodyina.
